Option D is the best and should be considered as the right answer! Cases like where "I cannot accept your Offer", it is to be considered that "Your offer cannot be accepted by ME". " ME" is not required to be mentioned. But yes, if any XYZ name was given instead of "I" then it is required to specify the name.

Option D is more appropriate than A. "By me " need not be mentioned as it is understood.
Relevant eg: active : People say he is very rich.
Passive : He is said to be very rich.
Here "by people " is not mentioned in the passive form. It is understood.

Option 'D' must have been correct if option 'A' was not given as a choice. Here the best answer is 'A'.
