Concrete/ graphite rods use as control rods and heavy water us as a moderator.

So heavy water is the right answer.

As per my knowledge the heavy water is used as a moderator to slow down the speed of neutrons making capable it for fission.

Heavy water is also used as a neutron moderator. Although cement and concrete are both the correct answer.

Graphite is commonly used as a moderator in nuclear power plant.

Heavy water is also used as a moderator although less frequently.

Concrete is commonly used as a shielding material.

Concrete is wrong. Heavy water and graphite are used as moderators.

It is used as shielding material to protect from radiation.
