Whenever damage of tissue occurs to the body, fibrinogen is released which causes formation of fibres at damaged place causing aggregation of platelets which in turn release prothrombin induced clot formation.

When vessels are damaged, the connective tissue around it is exposed to blood. Platelets are attracted to the connective tissue.
Prothrombin is released by the platelets that then helps in converting fibrinogen to fibrin. Fibrin now is the main clotting factor that forms some sort of net to clot the damage.

Two enzymes are involved in blood clotting. Prothrombinase and thrombokinase. Due to the action of the enzymes thrombin or blood clot is formed during injury.

When a individual acquires an injury, this activates platelets/thrombocytes to accumulate at the damaged site, their accumulation leads to the release of a protein substance known as thromboplastin, this along with vitamin K and calcium ions activates the enzyme prothrombin.
