Efficiency = Pout/ Pin.
Where Pout is the output power,
The pin is power without loses = Pout + Plosses.
The amount of internal Power Loss or Plosses is the major factor in determining the value of efficiency of a power supply.

This basically refers to power dissipation in the internal resistance of the power source.
