1) It is an active filter since opamp is used. In this resistor and then capacitor used, so we conclude that which is low pass filter.
2) It is second order since two combinations of resistor and capacitor used.
3) For first order filter gain is 20db produced by one pair of resistor and capacitor.
For second order filter gain is 40 db produced by two pair of resistor and capacitor.

This is a Sallen-Key second order low pass filter with gain.
