A second horizontal stiffener is always placed at the neutral axis of the girder if the thickness of the web is less than
d/250 for structural steel
d/225 for high tensile steel
both (c) and (b)

If d/tw≤85.
No stiffeners are required.

If 85≤d/tw≤200.
Vertical stiffeners are required.

If 200≤d/tw≤250
The Vertical stiffener and one horizontal stiffener is provided at a distance of 2/5 from the compression flange to the neutral

If 250≤d/tw≤400.
Plus one horizontal stiffener at neutral.

At support and concentrated load.
Bearing stiffeners are provided under concentrated load to avoid local bending of the flange as well as local buckling of the web.

D/t < 85εw: No stiffeners needed (unstiffened).
85εw < d/t < 200εw: Vertical stiffeners may suffice.
200εw < d/t < 250εw: Vertical + one horizontal stiffener (often at 0. 2d from compression flange).
D/t > 250εw: Requires vertical stiffeners plus an additional horizontal stiffener at the neutral axis.

So, option D is correct.
