Shear reinforcement means stirrups and it shouldn't greater than 0.8% of total reinforcement.

Cracks develop at 45 degree from D/2 distance from the face of support where D is the depth of the beam, flexural cracks occurs at middle of the beam with cracks at 90 degree.

Shear failure at section of beam without shear reinforcement will normally occur at 30 degree to the horizontal.

Its because tension reinforcement wants to expand, so the cracks are developed at 45 degree at the support.

It is towards the middle of the beam at the neutral axis. And at bottom tensile strength is along horizontal so cracks will be perpendicular.
