Width formation includes the width of pavement + shoulder.

Width of formation or roadway is the sum of width of pavement or carriageway including separators if any and the shoulders.

FORMATION OR ROADWAY width is the top width of highway embankment or bottom width of highway cutting excluding the side drains.

Because in Option B we are considering only about the pavement and shoulders, but if there is a separation in the road then we are not considering that. So, Option B is partially correct.

And Option D is correct for all the cases.
