Consider your mom is saying that the man's mother ie your mother is the only daughter of your maternal grandmother. Your mom doesn't have any sister she is referring to herself in an indirect way.

There is one more thing answer can be "sister" also...
She is the only daughter... But its not mentioned that she is the only child...
So there cn be a son also of that women...
Than answer will be sister...

In this question, the women is saying that the man is the son of only daughter of his mother. Here word only word has been used it means that the women's mother only daughter is the women herself. So it is clear that the man is the son the women.
