In past conditional sentence, we can write; If+sub+had+V3 or Had+sub+V3 so question and option C grammatically right but the question is a sentence improvement that's why the 2nd form is a more requisite sentence.

There is no improvement needed. So option D is the correct one.

"If" should be followed by "Then". As we don't have "Then" in the given sentence, we need to remove "If". Hence the answer: C.

If conditionals:
Type 1: If noun/prenoun+present tense, noun/pronoun+future tense.
Type 2: If noun/pronoun+past tense, noun/pronoun+would/could/might+simple verb.
Type 3: If noun/pronoun+past perfect, noun/pronoun+would/could+have+past participle.
So option C is the only right option. But isn't the original statement right too.

The question is choose the alternative so that we can improve. So we have to choose the answer if given. Both are correct.
