In past conditional sentence, we can write; If+sub+had+V3 or Had+sub+V3 so question and option C grammatically right but the question is a sentence improvement that's why the 2nd form is a more requisite sentence.

"If" should be followed by "Then". As we don't have "Then" in the given sentence, we need to remove "If". Hence the answer: C.
