Dear is something that you would cherish or something that is very close to you for a variety of reasons, it surely does not mean expensive. Hence, it's antonym cannot be cheap.

Why not worthless? please explain.

Worthless is right.

Example : Oh, this book is very dear to me.

I don't think "Cheap" fits the antonym here while "Worthless" makes perfect sense.

Dear are valuable to us and worthless means valueless. So why not worthless?

Dear is something really worth to a person. It does not show the cost of that particular thing.

So the opposite is inappropriate to be cheap. It must be worthless.
