We are designed it as a homogeneous material but it should be in nature it acts like as a heterogeneous material am I right?

It is heterogeneous but in design, it is assumed homogeneous.

Yes, it is right that's why we take a separate factor of safety for reinforcement and for concrete.

Yes, concrete is a heterogeneous mixture. Because of in mixture different ingredients are there (cement, sand, aggregates and water).
Homogeneous means only one material of mixture.

It's possible to physically separate components of a heterogeneous mixture.
Example: Soil, concrete etc.
While the homogeneous material is uniform through out and we can't see the individual chemicals or ingredients.
