Both A and C are possible because C applies for major instances such as an accident or a more virulent disease and A for a less significant disease where you pay the bills first for the consultation and medicines and then you recover at home.

I think this may be the wrong answer.
Correct answer should be 1 4 2 5 3.
As the patient goes to Doctor then Doctor diagnose the issue and prescribe the treatment and Bill is handed over to the patient for payment.
